O Allah, I seek refuge in You from the cessation of Your blessings, the turning of Your wellness, the suddenness of Your vengeance, and all of Your displeasure.
Allahumma inni a'udhu bika min zawali ni'matik, wa tahawwuli 'afiyatik, wa fuja'ati niqmatik, wa jami'i sakhatik

Source & Authentication
Narrated by Abdullah ibn Umar (may Allah be pleased with them both): The Messenger of Allah (peace be upon him) used to teach them to seek refuge (in Allah) from the changes in health, the suddenness of punishment, the evil of the decree, and the evil of the enemy. (He used to say: 'Allahumma inni a'udhu bika min zawali ni'matik, wa tahawwuli 'afiyatik, wa fuja'ati niqmatik, wa jami'i sakhatik.' [O Allah, I seek refuge in You from the cessation of Your blessings, the turning of Your wellness, the suddenness of Your vengeance, and all of Your displeasure.])
