اللَّهُمَّ إِنِّي أَسْأَلُكَ وَأَتَوَجَّهُ إِلَيْكَ بِنَبِيِّكَ مُحَمَّدٍ نَبِيِّ الرَّحْمَةِ، يَا مُحَمَّدُ إِنِّي قَدْ تَوَجَّهْتُ إِلَيْكَ بِرَبِّي فِي حَاجَتِي هَذِهِ لِتُقْضَى، اللَّهُمَّ فَشَفِّعْهُ فِيَّ اللَّهُمَّ عَافِنِي فِي بَدَنِي، اللَّهُمَّ عَافِنِي فِي سَمْعِي، اللَّهُمَّ عَافِنِي فِي بَصَرِي، لَا إِلَهَ إِلَّا أَنْتَ اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنَ الْكُفْرِ وَالْفَقْرِ، اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْ عَذَابِ الْقَبْرِ، لَا إِلَهَ إِلَّا أَنْتَ
O Allah, I ask You and turn to You through Your Prophet, Muhammad, the Prophet of mercy. O Muhammad, I have turned to my Lord through you concerning my need, so that it may be fulfilled for me. O Allah, accept his intercession for me. O Allah, grant me health in my body. O Allah, grant me health in my hearing. O Allah, grant me health in my sight. There is no god but You. O Allah, I seek refuge in You from disbelief and poverty. O Allah, I seek refuge in You from the punishment of the grave. There is no god but You.
Allahumma inni as'aluka wa atawajjahu ilayka bi nabiyyika Muhammadin nabiyyi ar-rahmah. Ya Muhammad, inni qad tawajjahtu ilayka bi Rabbi fi hajati hadhihi li tuqda. Allahumma fa shaffi'hu fiyya. Allahumma 'afini fi badani. Allahumma 'afini fi sam'i. Allahumma 'afini fi basari. La ilaha illa Anta. Allahumma inni a'udhu bika minal kufri wal faqr. Allahumma inni a'udhu bika min 'adhabi al-qabri. La ilaha illa Anta.

The companion, Uthman bin Hunaif (may Allah be pleased with him), narrated: A blind man came to the Prophet (peace be upon him) and said: 'Pray to Allah to grant me health.' The Prophet (peace be upon him) said: 'If you wish, you may be patient, and that would be better for you. If you wish, I shall supplicate for you.' The man said: 'Supplicate for me.' So the Prophet (peace be upon him) supplicated for him. The man then said: 'I ask You and turn to You through Your Prophet Muhammad, the Prophet of mercy. O Muhammad, I have turned to my Lord through you concerning my need, so that it may be fulfilled for me. O Allah, accept his intercession for me.' So the Prophet (peace be upon him) supplicated for him and it was as if they were sitting with a medicine tray laden with gold. (Sunan Abi Dawud 907 - graded Hasan by some scholars)
