اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْ زَوَالِ نِعْمَتِكَ، وَتَحَوُّلِ عَافِيَتِكَ، وَفُجَاءَةِ نِقْمَتِكَ، وَجَمِيعِ سَخَطِكَ، اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْهَا. رَبَّنَا أَفْرِغْ عَلَيْنَا صَبْرًا وَتَوَفَّنَا مُسْلِمِينَ
O Allah, I seek refuge in You from the decline of Your blessings, the change of Your protection, the suddenness of Your vengeance, and all of Your displeasure. O Allah, I seek refuge in You from them. Our Lord, bestow upon us patience and cause us to die as Muslims [in submission to You].
Allahumma inni a'udhu bika min zawali ni'matika, wa tahawwuli 'afiyatika, wa faja'ati niqmatik, wa jami'i sakhatik, Allahumma inni a'udhu bika minha. Rabbana afrigh 'alayna sabran wa tawaffana muslimeen.

The first part of the dua is found in a hadith narrated by 'Aishah (may Allah be pleased with her) who said: The Messenger of Allah (ﷺ) used to supplicate: 'O Allah, I seek refuge in You from the decline of Your blessings, the change of Your protection, the suddenness of Your vengeance, and all of Your displeasure.' (Sahih Muslim 1300) The second part is from the Quran, where the magicians of Pharaoh, after witnessing the miracle of Musa (peace be upon him), declared: 'Our Lord, bestow upon us patience and cause us to die as Muslims.' (Quran 7:126)
