اللَّهُمَّ اغْفِرْ لِي، وَارْحَمْنِي، وَاهْدِنِي، وَعَافِنِي، وَارْزُقْنِي.

O Allah, forgive me, have mercy upon me, guide me, grant me well-being, and provide for me.

Allahumma-ghfir li, warhamni, wahdini, wa 'afini, warzuqni.

The narration from Sahih Muslim reads: 'Umar ibn Al-Khattab (may Allah be pleased with him) reported: The Messenger of Allah ﷺ used to supplicate: 'O Allah, grant me well-being in my body. O Allah, grant me well-being in my hearing. O Allah, grant me well-being in my sight. There is no god but You. O Allah, I seek refuge in You from disbelief and poverty. O Allah, I seek refuge in You from the torment of the grave. There is no god but You.' (Sahih Muslim 2697). Another version, also found in Muslim, is: 'Allahumma inni as'alukal-'afwa wal-'afiyah fid-dunya wal-akhirah. Allahumma inni as'alukal-'afwa wal-'afiyah fi dini wa dunyaya wa ahli wa mali...' (O Allah, I ask You for pardon and well-being in this world and the Hereafter. O Allah, I ask You for pardon and well-being in my religion, my worldly affairs, my family and my wealth...). The version used above is a shorter, comprehensive form taught by the Prophet ﷺ, often recited after prayers.
