اَللّٰهُمَّ اَنْتَ خَلَقْتَنِيْ فَاَنْتَ تُقَوِّمُنِيْ، وَاَنْتَ تُطْعِمُنِيْ فَاَنْتَ تُسْقِيْنِيْ، وَاَنْتَ تُمِيْتُنِيْ فَاَنْتَ تُحْيِيْنِيْ، وَاَنْتَ خَلَقْتَ جِبِلِّيْ وَعِظَامِيْ، فَاَسْاَلُكَ اَنْ تُعِيْدَنِيْ مِنْ ذٰلِكَ اَللّٰهُمَّ اِنِّيْ اَسْاَلُكَ الْعَافِيَةَ فِي الدُّنْيَا وَالْاٰخِرَةِ

O Allah, You created me, and You guide me; You feed me and You give me to drink; You cause me to die and You give me life. You created my constitution and my bones. So I ask You to preserve me from that (i.e., from whatever You may decree). O Allah, I ask You for well-being in this world and the Hereafter.

Allahumma anta khalaqtani fa anta tuqawwimuni, wa anta tut'imuni fa anta tusqini, wa anta tumituni fa anta tuhyini, wa anta khalaqta jibili wa 'idhami, fa as'aluka an tu'idani min dhalik. Allahumma inni as'alukal-'afiyata fid-dunya wal-akhirah.

Narrated by Abdullah bin Amr (may Allah be pleased with him): A man asked the Messenger of Allah (peace be upon him): 'What should I ask for?' He said: 'Ask Allah for Al-Afiyah (well-being).' He stayed for a few days, then asked: 'O Messenger of Allah! What should I ask for?' He said: 'O Allah, grant me Al-Afiyah in my religion, my worldly affairs, my family and my wealth. O Allah, conceal my faults and grant me security from what I fear.' O Allah, protect me from before me and from behind me, from my right and from my left, and from above me, and I seek refuge in Your Might from being taken from beneath me.' (He said): 'And he said: 'Abdullah bin 'Umar (may Allah be pleased with him) used to say: 'This is a supplication that I have not neglected since I heard it from the Messenger of Allah (peace be upon him).'' (In another narration): It was narrated from Abdullah bin 'Amr (may Allah be pleased with him) that the Messenger of Allah (peace be upon him) said: 'You should ask Allah for pardon and well-being ('Afiyah) in this world and the Hereafter.
