اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْ زَوَالِ نِعْمَتِكَ، وَتَحَوُّلِ عَافِيَتِكَ، وَفُجْأَةِ نِقْمَتِكَ، وَجَمِيعِ سَخَطِكَ. اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْهَا.
O Allah, I seek refuge in You from the disappearance of Your blessings, and the reversal of the well-being You granted, and the suddenness of Your punishment, and all that You are displeased with. O Allah, I seek refuge in You from them.
Allahumma inni a'udhu bika min zawali ni'matika, wa tahawwuli 'afiyatika, wa fuj'ati niqmatika, wa jami'i sakhatika. Allahumma inni a'udhu bika minha.

Umm Salamah (may Allah be pleased with her) reported: Whenever the Prophet (ﷺ) said the salam at the end of the prayer, he would say: 'O Allah, I seek refuge in You from the shameful life and the shameful death, and from the evil of the Antichrist, and from the punishment of the grave.' He would say it twice or thrice. He would say: 'O Allah, I seek refuge in You from the disappearance of Your blessings, and the reversal of the well-being You granted, and the suddenness of Your punishment, and all that You are displeased with.' He would say: 'O Allah, I seek refuge in You from the evil of what I hear, and from the evil of what I see, and from the evil of what I say, and from the evil of what I do.' He would say: 'O Allah, I seek refuge in You from the going astray or being led astray, or from my slipping or being caused to slip, or from my wronging or being wronged, or from my acting foolishly or being acted upon foolishly.'
