اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنَ الْبَرَصِ، وَالْجُنُونِ، وَالْجُذَامِ، وَمِنْ سَيِّئِ الأَسْقَامِ.
O Allah, I seek refuge in You from lepros y, insanity, leprosy, and from the worst of diseases.
Allahumma inni a'udhu bika minal-barasi wal-jununi wal-judhami wa min sayyi'il-asqam.

The Prophet Muhammad ﷺ used to seek refuge in Allah from the evil of that which had been revealed to him. He used to say: 'O Allah, I seek refuge in You from the evil of what I know and the evil of what I do not know.' (Sahih Muslim 2708) And he also used to say: 'O Allah, I seek refuge in You from the punishment of Hellfire, and I seek refuge in You from the punishment of the grave, and I seek refuge in You from the trial of the Dajjal, and I seek refuge in You from the trial of life and the trial of death. O Allah, I seek refuge in You from sin and debt.' (Sahih al-Bukhari 6368) Regarding the specific dua for illness: Narrated by Abdullah bin Amir (may Allah be pleased with him): My father called me, and I was suffering from a fever. He said: 'I heard the Messenger of Allah ﷺ say: 'Indeed, fever is from the heat of Hellfire, so cool it down with water.' When he (Abdullah's father) mentioned the Prophet's saying about fever, my father mentioned the Prophet's saying: 'When one of you visits a sick person, he should supplicate for him three times, asking Allah to cure him from his illness.' (Sunan Abi Dawud 3107) Also related: Narrated by Uthman bin Affan (may Allah be pleased with him): The Messenger of Allah ﷺ said: 'Whoever says 'Bismillah alladhi la yadhurru ma'a ismihi shay'un fil-ardi wa la fis-sama'i wa huwas-Sami'ul-'Alim' (In the name of Allah, with Whose name nothing can cause harm in the earth nor in the heavens, and He is the All-Hearing, the All-Knowing) three times, will not be afflicted by sudden calamity until morning comes, and whoever says it in the morning will not be afflicted by sudden calamity until evening comes.' (Sunan Abi Dawud 5088 - Sahih)
