اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْ زَوَالِ نِعْمَتِكَ، وَتَحَوُّلِ عَافِيَتِكَ، وَفُجْأَةِ نِقْمَتِكَ، وَجَمِيعِ سَخَطِكَ، وَأَعُوذُ بِكَ مِنْ سُوءِ الْأَخْلَاقِ وَالْأَعْمَالِ وَالْأَهْوَاءِ.
O Allah, I seek refuge in You from the cessation of Your blessings, the change of Your well-being, the suddenness of Your punishment, and all that You decree. And I seek refuge in You from bad character, bad deeds, and evil desires.
Allahumma inni a'udhu bika min zawali ni'matik, wa tahawwuli 'afiyatik, wa fuja'ati niqmatik, wa jami'i sakhatik, wa a'udhu bika min su'il akhlaqi wal a'mali wal ahwa'.

It was narrated from Abu Hurairah that the Messenger of Allah (ﷺ) used to seek refuge in Allah from four things, saying: 'Allahumma inni a'udhu bika min zawali ni'matik, wa tahawwuli 'afiyatik, wa fuja'ati niqmatik, wa jami'i sakhatik.' (Then he mentioned two other things, which are not mentioned in this version). (Sahih Muslim 2708).
