O Allah, forgive them and have mercy on them, and pardon them and absolve them, and honour their reception, and make their entrance spacious, and cleanse them with water, snow, and hail, and purify them from sins as a white garment is purified from dirt, and give them a home better than their home, and a family better than their family, and a spouse better than their spouse, and admit them to Paradise, and protect them from the punishment of the grave and from the punishment of the Fire.

Source & Authentication
When the Prophet Muhammad (peace be upon him) prayed over a deceased person, he would say: 'O Allah, forgive our living and our dead, those who are present among us and those who are absent, our young and our old, our males and our females. O Allah, whoever you keep alive among us, keep him alive in Islam, and whoever you cause to die among us, cause him to die in faith. O Allah, do not deprive us of its (i.e. the reward of the deceased's patience) reward and do not cause us to go astray after him.' (Sahih Muslim 963) The text provided in the entry is a more detailed version attributed to the Prophet (peace be upon him) in other narrations that are often recited in the context of praying for the deceased or visiting mourners, such as in Abu Dawud and Ibn Majah, though Sahih Muslim 963 is a widely accepted basis for this type of supplication.
