اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْ زَوَالِ نِعْمَتِكَ، وَتَحَوُّلِ عَافِيَتِكَ، وَفَجْأَةِ نِقْمَتِكَ، وَجَمِيعِ سَخَطِكَ، وَأَعُوذُ بِكَ مِنَ الْهَدْمِ، وَأَعُوذُ بِكَ مِنَ التَّرَدِّي، وَأَعُوذُ بِكَ مِنَ الْغَرَقِ، وَالْحَرَقِ، وَالْهَرَمِ، وَأَعُوذُ بِكَ أَنْ يَتَخَبَّطَنِي الشَّيْطَانُ عِنْدَ الْمَوْتِ، وَأَعُوذُ بِكَ أَنْ أَمُوتَ فِي سَبِيلِكَ مُدْبِرًا، وَأَعُوذُ بِكَ أَنْ أَمُوتَ لَدِيغًا.
O Allah, I seek refuge in You from the passing away of Your favors, and from the change of Your protection, and from the suddenness of Your punishment, and from all that You are displeased with. I seek refuge in You from being demolished, I seek refuge in You from falling from a height, I seek refuge in You from drowning, burning, and old age. I seek refuge in You from being misled by Satan at the time of death. I seek refuge in You from dying in Your cause while turning my back. I seek refuge in You from dying from a venomous bite.
Allahumma inni a'udhu bika min zawaali ni'matika, wa tahawwuli 'aafiyatika, wa faj'ati niqmatika, wa jamee'i sakhatika. Wa a'udhu bika minal hadm, wa a'udhu bika minat taraddi, wa a'udhu bika minal gharaqi, wal haraqi, wal harami. Wa a'udhu bika an yatakhabbatani-sh-shaytaanu 'indal mawt, wa a'udhu bika an amoota fee sabeelika mudbiran, wa a'udhu bika an amoota ladeeghan.

It was narrated from 'Abdullah bin 'Umar (may Allah be pleased with them) that the Messenger of Allah ﷺ used to supplicate with these words: 'Allahumma inni a'udhu bika min zawaali ni'matika, wa tahawwuli 'aafiyatika, wa faj'ati niqmatika, wa jamee'i sakhatika. Wa a'udhu bika minal hadm, wa a'udhu bika minat taraddi, wa a'udhu bika minal gharaqi, wal haraqi, wal harami.
