اللَّهُمَّ اغْفِرْ لِفُلانٍ، وَارْحَمْهُ، وَعَافِهِ، وَاعْفُ عَنْهُ، وَأَكْرِمْ نُزُلَهُ، وَوَسِّعْ مَدْخَلَهُ، وَاغْسِلْهُ بِالْمَاءِ وَالثَّلْجِ وَالْبَرَدِ، وَنَقِّهِ مِنَ الْخَطَايَا كَمَا يُنَقَّى الثَّوْبُ الأَبْيَضُ مِنَ الدَّنَسِ، وَأَبْدِلْهُ دَارًا خَيْرًا مِنْ دَارِهِ، وَأَهْلًا خَيْرًا مِنْ أَهْلِهِ، وَزَوْجًا خَيْرًا مِنْ زَوْجِهِ، وَأَدْخِلْهُ الْجَنَّةَ، وَأَعِذْهُ مِنْ عَذَابِ الْقَبْرِ وَعَذَابِ النَّارِ
O Allah, forgive [Name of the deceased], have mercy upon him, grant him well-being, pardon him, make his resting place honorable, widen his entrance, cleanse him with water, snow, and hail. Purify him from sins just as a white garment is purified from dirt. Replace his home with a better home, his family with a better family, and his spouse with a better spouse. Admit him to Paradise, and protect him from the punishment of the grave and the punishment of the Fire.
Allahummaghfir li [Fulan], warhamhu, wa 'aafihi, wa'fu 'anhu, wa akrim nuzulahu, wa wassi' madkhalahu, waghsilhu bil-ma'i wath-thalji wal-baradi, wa naqqihi minal-khataaya kamaa yunqqath-thawbul-abyadu minad-danasi, wa abdilhu daaran khayran min daarih, wa ahlan khayran min ahlih, wa zawjan khayran min zawjih, wa adkhilhul-jannah, wa a'idh-hu min 'adhaabil-qabri wa 'adhaabin-naar.

Recite it during funeral prayers, upon visiting the grave, or anytime you wish to make sincere dua for their departed soul.
How to Read
Make the intention (niyyah) for the deceased. Replace '[Fulan]' with the name of the person you are making dua for. If you don't know their name or are reciting generally, you can omit it and the general meaning still applies. Recite with sincerity and focus, remembering the blessings Allah bestowed upon the deceased and their good deeds, including their mercy towards animals.

The Prophet Muhammad (peace be upon him) said, 'When you offer the funeral prayer over a dead person, say: O Allah, forgive him, have mercy on him, grant him well-being, pardon him, make his lodging honorable, give ample space to him as he enters, wash him with water and snow and hail, cleanse him from his sins as a white garment is cleansed from dirt, and give him in exchange a home better than his home, a family better than his family, and a wife better than his wife, and admit him to Paradise and guard him against the torment of the grave.'
