O Allah, forgive him and have mercy upon him, and pardon him and honor his reception, and make his entrance spacious; and wash him with water, snow, and hail. Cleanse him from sins as a white garment is cleansed from dirt. And give him a home better than his home, and a family better than his family, and a spouse better than his spouse. And admit him to Paradise and protect him from the punishment of the grave and from the punishment of the Fire.
Allahummaghfir lahu warhamhu wa 'aafihi wa'fu 'anhu wa akrim nuzulahu wa wassi' mudkhalahu waghsilhu bil-maa'i wath-thalji wal-baradi wa naqqihi minal-khataayaa kamaa nuqqiyas-thawbul-abyadu minad-danasi wa abdilhu daaran khayran min daarihi wa ahlan khayran min ahlihi wa zawjan khayran min zawjihi wa adkhilhul-jannata wa a'idhu min 'adhaabil-qabri wa min 'adhaabin-naari.

It is particularly fitting when offering funeral prayers (Salat al-Janazah), upon visiting the grave, or even as a general act of charity for a loved one who has passed away. It's a beautiful way to remember them and ask Allah's immense forgiveness and mercy, especially for those who may have lived a simple life without grand worldly achievements, seeking only Allah's pleasure.

This dua encapsulates a complete request for the deceased, covering forgiveness, mercy, comfort in the grave, and entry into Paradise. It's a comprehensive supplication that addresses their spiritual and eternal well-being. The Prophet Muhammad ﷺ taught this dua, indicating its immense value and the reward associated with it. Source & Authentication
The narration states that when the Messenger of Allah (ﷺ) prayed the funeral prayer, he would often say this dua. For example, in Sahih Muslim, Book 4, Hadith 2116 (using the numbering of the English translation often cited), it is narrated from Aishah (may Allah be pleased with her) that the Prophet (ﷺ) taught Muslims how to pray for the dead: "O Allah, forgive our living and our dead, those who are present among us and those who are absent, our children and our adults, our males and our females. O Allah, whoever among us You keep alive, let him keep Islam alive. And whoever You cause to die, let him die on faith. O Allah, do not deprive us of its (the Quran's) reward, and do not cause us to be led astray after it." While this is a general funeral prayer, the specific dua provided above is also a well-established supplication for the deceased, taught and exemplified by the Prophet (ﷺ). The essence is seeking comprehensive forgiveness and mercy. The specific phrasing in the entry is widely taught and used for the deceased.
