It's a recognition that even the act of eating, a fundamental necessity, is an act of worship when done with the intention of seeking Allah's blessing and strength, particularly for a sick individual trying to regain their health. Saying 'Bismillah' before eating is a Sunnah that brings Allah's blessings to the food and to the person consuming it. For a sick person, this blessing is even more vital. It reminds them that their healing is in Allah's hands and that every nourishment is a means of recovery sent by Him. The Prophet Muhammad ﷺ said: 'When any of you eats something, he should say: 'Bismillah'. If he forgets to say it before starting, he should say when he remembers: 'Bismillah awwalahu wa akhirahu' (In the name of Allah at the beginning and at the end).'' (Sunan Abi Dawud 3767). This hadith, though general, underscores the importance of this utterance for all occasions, including when one is unwell and seeking strength from their food.
