اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنَ الْبَرَصِ وَالْجُنُونِ وَالْجُذَامِ وَمِنْ سَيِّئِ الأَسْقَامِ

O Allah, I seek refuge in You from leprosy, madness, elephantiasis, and from all bad diseases.

Allahumma inni a'udhu bika minal-barasi wal-jununi wal-judhami wa min sayyi'il-asqam

Narrated by 'Abdullah ibn 'Amr (may Allah be pleased with him) that the Messenger of Allah ﷺ used to teach them this invocation as he used to teach them a Surah of the Qur'an: 'O Allah, I seek refuge in You from the denial of blessings, the change of well-being, the sudden affliction, and all Your wrath.' He would say: 'Indeed, from it are the denial of blessings, the change of well-being, the sudden affliction, and all Your wrath.' (The narrator then mentioned the wording which is translated above).
