O Allah, I seek Your guidance [through Your knowledge], and I seek ability [to do what is best] through Your power, and I ask You for Your immense bounty. For indeed You have power, and I have no power; You have knowledge, and I have no knowledge; and You are the Knower of the unseen. O Allah, if You know that this matter (mention your need here) is good for me in my religion, my worldly life, and my Hereafter [or: in my present and future affairs], then decree it for me, make it easy for me, and bless it for me. And if You know that this matter is not good for me, then turn it away from me, and turn me away from it, and decree for me whatever is good, and make me satisfied with it.
Allahumma inni astakhiruka bi 'ilmika, wa astaqdiruka bi qudratika, wa as'aluka min fadlikal-'adheem. Fa innaka taqdiru wa la aqdiru, wa ta'lamu wa la a'lamu, wa anta 'allamul-ghuyoob. Allahumma in kunta ta'lamu anna hadhal-amra (here you mention your need) khayrun li fi deeni wa ma'eeshati wa 'aaqibati amri - aw qala: 'aajili amri wa aajilihi - faqdurnee lee, wa yassirhu lee, thumma barik lee feehi. Wa in kunta ta'lamu anna hadhal-amra sharrun li fi deeni wa ma'eeshati wa 'aaqibati amri - aw qala: 'aajili amri wa aajilihi - fasrifhu 'annee, wasrifni 'anhu, waqdurlil-khaira haithu kaana, thumma radhini bihi.
When to Read
This dua is specifically for seeking Allah's guidance when faced with a decision, especially when rebuilding trust after betrayal. How to Read
Perform two rak'ahs of voluntary prayer (Salatul Istikhara). After the tasleem (conclusion of the prayer), recite this dua. Clearly state the matter you are seeking guidance on when you reach the part where it says '(mention your need here)'. For example, you would say: 'Allahumma inni astakhiruka li himmi islah al-thiqah ma'a fulan...' (O Allah, I seek Your guidance for the matter of restoring trust with so-and-so...). Focus your intention and heart on seeking Allah's wisdom. After reciting the dua, sleep in a state of readiness to accept Allah's guidance, which may come through dreams, clarity in your heart, or circumstances unfolding in a particular way.

Jabir bin 'Abdullah (may Allah be pleased with him) said: The Prophet (peace be upon him) used to teach us the Istikhara in all matters as he used to teach us the Surah from the Quran. He said: 'If anyone of you is concerned about a matter he should pray two rak'ahs of non-obligatory prayer, and then say: O Allah, I seek Your guidance through Your knowledge, and I seek ability through Your power, and I ask You of Your immense bounty, for You have power, and I have not, and You have knowledge, and I know not the unseen. O Allah, if You know that this matter (mention your need here) is good for me in my religion, my worldly life, and my Hereafter (or: in my present and future affairs), then decree it for me, make it easy for me, and then bless it for me. And if You know that this matter is bad for me in my religion, my worldly life, and my Hereafter (or: in my present and future affairs), then turn it away from me, and turn me away from it, and decree for me whatever is good, and make me satisfied with it.
